Life Care Center at South Mountain offers skilled nursing and long-term care services and is centrally located in Phoenix, Arizona. We offer physical, occupational, speech, intravenous and wound therapy through care plans that are individualized according to each resident's specific needs.

In 2009, our Community was honored with national recognition from the American Health Care Association for Quality Improvement efforts. The Arizona Department of Health Services has also awarded Life Care Center at South Mountain the elusive "Excellent and A" rating! Not only has our Community been locally and nationally recognized, we are also proud to be home to the 2009 Arizona Healthcare Professional of the Year (Joseph Castillo, Wound Care Certified Director of Nursing) as well as the 2009 Medical Director of the Year (Dr. Kenneth Pettit, DO).
